The PACE Model of Care The Program of All-Inclusive Care for the Elderly (PACE) model of care is designed to help older adults remain in their home in the community for as long as possible. The PACE model provides both medical and social services and is a welcome alternative to nursing home care for many older adults.
The PACE provides medical and social services for people with significant needs who want to continue living at home. It’s a combined effort between Medicare and Medicaid.
WHAT IS PACE? The Program of All-Inclusive Care for the Elderly (PACE) helps seniors with complex medical needs. The PACE program helps seniors avoid nursing homes and instead receive the care and services necessary to help them be safe, comfortable and healthy while living in their own community.
To participate in PACE, an individual must be 55 years of age or older, require nursing home level of care but be able to live safely in the community at time of enrollment with the services of PACE, and reside in the service area of a PACE organization.
